Since we opened our doors in 2007, Elite Sports Physical Therapy has provided physical therapy services to over 5,000 individuals. Our featured services include sports therapy programs for dancers, swimmers, martial artists and team sports, walking and balance programs, spine care, and post-operative rehabilitation.
We believe that it is the responsibility of the entire ESPT team to make each of our patients’ rehabilitation experiences a success. Starting with our company culture and lively atmosphere, we hope that each of our patients can be motivated and inspired to achieve their best possible health.
Requesting an Appointment for Examination
We are currently accepting new patients and typically have less than a one-week waiting period for the next available appointment. In order to request an appointment, please have the following information available:
- Demographic information including date of birth, home address, phone number, email address
- Insurance information, if applicable, to include group and policy number
- Your doctor’s referral/prescription for physical therapy, if available
- Multiple Dates/Times interested
What to Expect
When Requesting an Appointment for Examination
Once you call our office to request an appointment, we will collect your demographic and insurance information and schedule a time for your first appointment. Between your call and your first visit, our patient care coordinator, Cherise, will check your physical therapy benefits to identify your coverage for physical therapy, if applicable. When You Arrive to the Office
We look forward to welcoming you to our clinic and understanding how best we can help you reach your health goals. At your first appointment please expect to arrive 30 minutes early to complete registration paperwork, if not completed at home. Please dress in appropriate clothing which can expose the body region to be examined, we recommend loose-fit clothing.
During your Initial Examination with the Physical Therapist
Your physical therapist will start with a “patient interview” in which we ask questions and learn about your current complaints, symptoms, activity limitations, and get an understanding of the timeline of events which brings us to the current day. Following, we run a series of examination procedures to evaluate your posture, movement pattern, range of motion & flexibility, strength & stability and assess the tissues & joints surrounding your area of pain. Following the examination portion of your session, your therapist will talk to you about a plan of care/course of treatment which will include a variability of the following: therapeutic exercise, manual therapy, posture/body mechanic re-training, and return to sports progression, if applicable.
During Your Course of Treatment at ESPT
We expect 100% commitment from our patients and this allows for the best outcomes. Your physical therapist will talk to you about the plan of care and course of treatment to help you reach your health goals. Throughout your sessions, you and your physical therapist will talk about your current gains/improvements and current limitations which helps your PT identify progress and adjust your plan of care according to your physical presentation that day. You will expect a variability of therapeutic exercise, manual therapy, posture/body mechanic re-training and graded progression to your goals.
About Billing & Insurance of Your Office Visits
As a courtesy to all our patients, we bill the insurance companies for you. A physical therapy claim (i.e., bill) is created at each attended appointment by your PT, processed within our internal billing department and then sent out to your insurance company. Once received by your insurance company, your insurance company will review the billing claim and process it within their department and determine the amount that is covered. The amount covered is determined by your individual PT benefits in which the remaining balance is billed to you later. Every insurance company processes billing at a different time schedule. Due to the nature of this process, you will expect a delay in receiving your statement balance from ESPT.
