Our Location
We are located at 194 Franciscan Lane, Suite 104, Fremont, CA, inside the Fremont Town Center. Our clinic is on the second floor of the building directly above the Warm Springs Optometric Group.
The Franciscan Shopping Plaza includes Safeway, Burger King, Taco Bell, USPS and FedEx —we are in the building situated near the entrance of the plaza, just a short walk from these landmarks.
Accessibility Note:
Our clinic is located on the second floor of the building. Elevator access is available just to the right of the main entrance to Pratim Kulkarni, D.D.S. (the dentist’s office located on the ground floor center of the building).
Stairwell access is available to the left side of the building next to the Optometrists.
If you need help finding us or assistance getting to your appointment, please call us when you arrive, and a team member will be happy to assist you.
Parking: Free parking is available in the plaza.
Disabled Parking: For easier access, we recommend parking near FedEx, where disabled parking spots are located and closest to the building’s elevator entrance.

From 880:
Traveling on 880S towards San Jose, take the Mission/Warren Ave. exit. Stay to the left of the fork and take the overpass towards Sacramento/Interstate 680. Before you reach the stop light, stay in the far right driving lane. After passing the first stop light (Warm Springs Blvd. and Mission Blvd.) – turn right into the driveway immediately after Burger King and before Taco Bell to enter the Safeway shopping plaza. Drive straight through the parking lot and the clinic will be on your left hand side past the KFC drive-thu on the second floor of the brown building right above Warm Springs Optometric Group.
From 680S (Mission):
Traveling on 680S towards San Jose, take Exit 12 for Mission Blvd. West and merge onto Mission Blvd. and stay in the far left driving lane. At the first stop light on Mojave Drive, turn left (towards gas station). Make the first right into Winema Common Shopping plaza after 7-eleven. Once you pass two speed bumps, make a left after KFC and the clinic will be on the second floor of the brown building right above Warm Springs Optometric Group.
From 680S (Scott Creek):
Traveling on 680S towards San Jose, take Exit 10 for Scott Creek Rd. and make a right to stay on Scott Creek Rd. Stay in the right lane and make a right turn onto Warm Springs Blvd. immediately after Cedar Lawn Cemetery and continue in the right lane for approximately 2 miles. When you see the 76 gas station on your right hand side, make a right turn into the plaza and drive past Safeway on your right and head towards the brown building. Upon approaching the brown building, the clinic will be on the second floor right above Warm Springs Optometric Group.
From 680S (Auto Mall Pkwy):
Traveling on 680S towards San Jose, take Exit 14 for Auto Mall Pkwy. and make a left onto Auto Mall Pkwy. Immediately, use the left to lanes to make a left onto Osgood Rd. and continue for approximately 2 miles. When you see Starbucks and Denny’s on your left hand side, use the second from the left lane to make a left turn onto Mission Blvd. After making the turn, turn right into the driveway immediately after Burger King and before Taco Bell to enter the Safeway shopping plaza. Drive straight through the parking lot and the clinic will be on your left hand side past the KFC drive-thu on the second floor of the brown building right above Warm Springs Optometric Group.
From 680N (Mission, Scott Creek Exit Closed):
Traveling on 680N towards Sunol, take Exit 12 for Mission Blvd. East and make an immediate right turn onto Curtner Rd. and continue for approximately half a mile. Make a right onto East Warren Ave. and continue for approximately a mile and when passing the Warm Springs Church on the left side, make a right turn onto Warm Springs Blvd. Stay in the right lane until you see the 76 gas station on your right hand side. Make a right turn into the plaza and drive past Safeway on your right and head towards the brown building. Upon approaching the brown building, the clinic will be on the second floor right above Warm Springs Optometric Group.
