Since we opened our doors in 2007, Elite Sports Physical Therapy has provided physical therapy services to over 5,000 individuals. Our featured services include sports therapy programs for dancers, swimmers, martial artists and team sports, walking and balance programs, spine care, and post-operative rehabilitation.

Requesting an Appointment for Examination

We are currently accepting new patients and typically have less than a one-week waiting period for the next available appointment. In order to request an appointment, please have the following information available:

  • Demographic information including date of birth, home address, phone number, email address
  • Insurance information, if applicable, to include group and policy number
  • Your doctor’s referral/prescription for physical therapy, if available
  • Multiple Dates/Times interested

What to Expect

When Requesting an Appointment for Examination

Once you call our office to request an appointment, we will collect your demographic and insurance information and schedule a time for your first appointment. Between your call and your first visit, our patient care coordinator, Cherise, will check your physical therapy benefits to identify your coverage for physical therapy, if applicable. Cherise will also email the New Patient Intake forms to complete your registration.

When You Arrive to the Office

We look forward to welcoming you to our clinic and understanding how best we can help you reach your health goals. At your first appointment please expect to arrive 30 minutes early to complete registration paperwork, if not completed at home. Please dress in appropriate clothing which can expose the body region to be examined, we recommend loose-fit clothing.

During your Initial Examination with the Physical Therapist

Your physical therapist will start with a “patient interview” in which we ask questions and learn about your current complaints, symptoms, activity limitations, and get an understanding of the timeline of events which brings us to the current day. Following, we run a series of examination procedures to evaluate your posture, movement pattern, range of motion & flexibility, strength & stability and assess the tissues & joints surrounding your area of pain. Following the examination portion of your session, your therapist will talk to you about a plan of care/course of treatment which will include a variability of the following: therapeutic exercise, manual therapy, posture/body mechanic re-training, and return to sports progression, if applicable.

During Your Course of Treatment at ESPT

We expect 100% commitment from our patients and this allows for the best outcomes. Your physical therapist will talk to you about the plan of care and course of treatment to help you reach your health goals. Throughout your sessions, you and your physical therapist will talk about your current gains/improvements and current limitations which helps your PT identify progress and adjust your plan of care according to your physical presentation that day. You will expect a variability of therapeutic exercise, manual therapy, posture/body mechanic re-training and graded progression to your goals.

About Billing & Insurance of Your Office Visits

As a courtesy to all our patients, we bill the insurance companies for you. A physical therapy claim (i.e., bill) is created at each attended appointment by your PT, processed within our internal billing department and then sent out to your insurance company. Once received by your insurance company, your insurance company will review the billing claim and process it within their department and determine the amount that is covered. The amount covered is determined by your individual PT benefits in which the remaining balance is billed to you later. Every insurance company processes billing at a different time schedule. Due to the nature of this process, you will expect a delay in receiving your statement balance from ESPT.

Billing Practices & Legal Compliance

At Elite Sports Physical Therapy, all services are billed according to federally recognized industry standards to ensure full compliance with insurance and Medicare regulations. For all time-based physical therapy services, we follow the Rule of 8s, which is the standard method used to determine how many billable units are supported based on the total minutes of direct skilled treatment provided. For Medicare beneficiaries, we strictly adhere to the Medicare 8-Minute Rule, which outlines specific time thresholds required for billing each unit of covered therapy.

We take billing accuracy seriously. This means that every treatment session—whether provided by a licensed physical therapist or a supervised student physical therapist—is billed in accordance with these national standards. Supervised student-provided care is legally billable when performed under the direct supervision of a licensed physical therapist, as permitted by CMS and private insurers.

Understanding Common CPT Codes and Billable Services

Physical therapy services are billed using standardized CPT (Current Procedural Terminology) codes that describe the type of care you receive. Below are some of the most commonly used codes in our clinic:

  • 97110 – Therapeutic Exercise: Activities focused on improving strength, range of motion, flexibility, and endurance. This may include use of weights, resistance bands, or bodyweight exercises.
  • 97530 – Therapeutic Activity: Functional movement training designed to improve coordination, balance, lifting, and task-specific skills that translate to daily activities (e.g., getting out of a chair, reaching, or squatting).
  • 97112 – Neuromuscular Re-education: Exercises or activities targeting balance, posture, coordination, and proprioception to help retrain the body’s movement systems.
  • 97116 – Gait Training: Treatment to improve walking mechanics, safety, and mobility using skilled instruction, assistive devices, or environmental adaptation.
  • 97140 – Manual Therapy: Hands-on techniques performed by the therapist such as soft tissue mobilization, joint mobilization, or passive stretching to relieve pain, restore motion, and improve function.

In addition to these procedures, time spent on medically necessary history taking, patient education, assessing your condition, modifying your plan of care, and making clinical decisions is considered part of your skilled treatment and is legally billable. These activities are essential components of your care and contribute directly to your progress and safety.

All services—whether provided by a licensed therapist or a supervised student therapist—are billed in accordance with federal, state, and payer-specific regulations. If you have questions about your visit or the codes listed on your invoice, our staff is happy to explain.
